Pemvidutide Dosage, half-life & reference guide

Also known as ALT-801

An investigational once-weekly GLP-1 and glucagon dual agonist from Altimmune, built for a gentler start: its two lower doses were given from the first injection without a dose ramp. In a 48-week phase 2 obesity trial, average weight loss reached 15.6% on the top dose (2.4 mg), and in people with fatty liver it cut liver fat by about half or more within 12 to 24 weeks. Its developer now develops it mainly for liver disease, with a phase 3 trial that began in 2026.

Pemvidutide dose guide

How it works.

Once-weekly GLP-1 and glucagon dual agonist from Altimmune, balanced 1:1 between the two receptors. Its EuPort domain binds albumin to slow its clearance for weekly dosing and, the company says, slows its entry into the blood, which it links to its tolerability. The GLP-1 arm curbs appetite; the glucagon arm acts directly on the liver, where it raises fat burning and curbs fat-making, and its developer now runs it mainly for liver disease. Investigational: not approved anywhere.

Commonly reported: Very little yet; it is rarely run outside trials.

  1. Day 1–84 · 1.2 mgRCT

    No ramp at this dose

In its trials, 1.2 mg was given from the first injection with no titration. Nausea was the main side effect (about 1 in 4, vs 1 in 9 on placebo), and stopping for side effects was no more common than on placebo. In the 48-week obesity trial, 1.2 mg averaged 10.3% weight loss.

    The science

MOMENTUM (Altimmune 2023; 391 adults with obesity or overweight, no diabetes; company-reported): 1.2 mg without titration; nausea 25.5% vs 11.3% on placebo, vomiting 6.1% vs 3.1%; side effects led 5.1% to stop vs 6.2%; −10.3% at 48 weeks vs −2.2%. In IMPACT (Noureddin 2025), 1.2 mg was also started without titration and no one on it stopped for side effects over 24 weeks.

  2. Day 1–84 · 1.8 mgRCT

    Most side effects come early

1.8 mg also starts without a ramp, and GI side effects are common early: in MOMENTUM about 6 in 10 had nausea and about 1 in 4 vomited, mostly mild to moderate, and most who stopped did so in the first 16 weeks. Average loss on 1.8 mg was 11.2% at 48 weeks.

    The science

MOMENTUM 1.8 mg: nausea 59.6%, vomiting 27.3%, diarrhea 10.1%, constipation 13.1%; side effects led 19.2% to stop vs 6.2% on placebo; −11.2% at 48 weeks (Altimmune 2023). In IMPACT's people with MASH, fewer stopped on the same dose: 1.2% over 48 weeks vs 3.5% on placebo, with 7.5% weight loss (Altimmune 2025). It gave the largest 12-week liver-fat drop in a fatty-liver trial, −68.5% (Harrison 2025).

  3. Day 1–112 · 2.4 mgRCT

The top dose, after a short ramp

2.4 mg is the highest dose tested, reached after a short titration. In MOMENTUM it averaged 15.6% loss at 48 weeks and was still falling, with about half of those analyzed at week 48 losing 15% or more. Nausea (about half) and vomiting (about 1 in 4) were the common side effects, mostly early.

    The science

MOMENTUM 2.4 mg (4-week titration): −15.6% at 48 weeks vs −2.2%, on a near-linear trajectory at the end; in the release's week-48 responder analysis (56 people on 2.4 mg), 51.8% reached at least 15% loss and 32.1% at least 20%. Nausea 51.5%, vomiting 27.8%, diarrhea 18.6%, constipation 22.7%; 19.6% stopped for side effects (Altimmune 2023). A 2026 trial reached 2.4 mg with a new two-step titration that the company says may be gentler on the gut (Altimmune 2026).

    What Pemvidutide is built on

Randomized trials in about 800 people: MOMENTUM in obesity (391, reported by the company, not yet in a journal), IMPACT in MASH (212, in The Lancet), and smaller fatty-liver and alcohol-use trials. It isn't approved anywhere; a phase 3 MASH trial began in 2026.

    The science

MOMENTUM (Altimmune 2023): −10.3%, −11.2% and −15.6% at 48 weeks on 1.2, 1.8 and 2.4 mg vs −2.2%. IMPACT (Noureddin 2025, Lancet; MASH with F2–F3 fibrosis): at 24 weeks, MASH resolution without worse fibrosis in 58% (1.2 mg) and 52% (1.8 mg) vs 20%, but fibrosis improvement didn't differ from placebo (33% and 36% vs 28%); at 48 weeks, non-invasive fibrosis markers improved vs placebo (company topline; Altimmune 2025). A fatty-liver trial randomized 94 people (Harrison 2025), and a 2026 trial in alcohol use disorder randomized 100 (company topline; Altimmune 2026). FDA granted Breakthrough Therapy designation for MASH; the phase 3 PERFORMA trial started in August 2026.

  5. With BPC-157Community

Why people add BPC-157 to Pemvidutide

GI side effects are pemvidutide's main side effects, and BPC-157, first studied as a stomach-protecting peptide, is added to ease them. Commonly reported with other GLP-1s: smoother step-ups. No trial has tested the pair.

    The science

BPC-157's early research was on gastric protection in animal studies (Sikiric 2018 review). No trial has tested it alongside pemvidutide or any GLP-1 drug; the pairing comes from community reports.

After week 8

  1. Day 57–168RCT

Liver fat drops faster than weight

In fatty-liver trials, pemvidutide cut liver fat by roughly half to three-quarters within 12 to 24 weeks, while weight fell by about 4 to 6%. The glucagon arm works on the liver directly, beyond what weight loss alone does.

    The science

Harrison 2025 (J Hepatol; 94 adults with liver fat of 10% or more): relative liver-fat drops at 12 weeks of 46.6%, 68.5% and 57.1% on 1.2, 1.8 and 2.4 mg vs 4.4% on placebo, with weight −4.3% on 1.8 mg. The 12-week extension (Browne 2025; 64 participants) reached 56.3%, 75.2% and 76.4% at 24 weeks vs 14.0%, with 53.8% on 1.8 mg reaching normal liver fat (5% or less), and weight fell 6.2%. The trial authors attribute the liver effect to glucagon receptor agonists acting directly on the liver to raise fat oxidation and inhibit fat-making.

  2. Day 169–336RCT

Most of the loss is fat

In MOMENTUM's MRI body-composition analysis, about three-quarters of the weight lost was fat and a quarter lean mass. Protein and resistance training are the levers people use to protect the lean part.

    The science

An MRI body-composition analysis from MOMENTUM found 74.5% of weight lost was adipose tissue and 25.5% lean mass (Altimmune 2024). In IMPACT's people with MASH, weight was still falling at 48 weeks on 1.8 mg (−7.5% vs −0.2% on placebo; Altimmune 2025).

Interactions

No interaction results have been published (a drug-interaction study in healthy volunteers, NCT04972396, is registered as completed). Approved GLP-1 drugs slow stomach emptying, which can change how oral medicines are absorbed, and add to the glucose-lowering of insulin or sulfonylureas; pemvidutide hasn't been characterized for either. In MOMENTUM, fasting glucose and HbA1c didn't change in people without diabetes. It is a GLP-1 drug itself, so it is not stacked with another one. Discuss all your medicines with your clinician.

Safety notes

Pemvidutide is investigational and has no label yet. In the 48-week MOMENTUM obesity trial (391 adults without diabetes; results reported by the company), nausea affected 25.5%, 59.6% and 51.5% on 1.2, 1.8 and 2.4 mg vs 11.3% on placebo, and vomiting 6.1%, 27.3% and 27.8% vs 3.1%, mostly mild to moderate; side effects led 5.1%, 19.2% and 19.6% to stop vs 6.2% on placebo, most within the first 16 weeks. There were no major cardiac events, cardiac side effects matched placebo, and average heart rate rose by about 3 beats per minute at most. In the IMPACT trial in people with MASH, 0% (1.2 mg) and 1.2% (1.8 mg) stopped for side effects over 48 weeks vs 3.5% on placebo. Until it has a label, the approved GLP-1 class cautions (thyroid C-cell tumors in rodents, pancreatitis, gallbladder disease) are the reference point. Discuss with your clinician.
