I missed my GLP-1 dose. Now what?

Each label has its own rule. Ozempic: take it within 5 days, after that skip it. Mounjaro and Zepbound: within 4 days (96 hours), after that skip it. Wegovy: take it if your next dose is more than 2 days away, skip it if it's less. Then keep your usual day.

Ozempic’s label

Inside the 5-day window

3 days late. Ozempic’s label: take it as soon as possible, then keep your usual day; the next dose stays on Mon, Oct 26, 2026.

“If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day. In each case, patients can then resume their regular once-weekly dosing schedule.”

Ozempic’s FDA label, section 2.1
In PepVaultPepVault sends a reminder on your dose day, and the Today screen shows how many hours late a dose is if the day slips.

The rule, label by label

Each shot’s FDA label has its own missed-dose rule, and they differ. Ozempic’s counts 5 days from the missed dose, Mounjaro’s and Zepbound’s 4 days (96 hours). Wegovy’s counts the other way, to the next dose: more than 2 days away, take it; less than 2 days, skip it. Every label then goes back to the usual weekly day.

“If one dose of WEGOVY injection is missed and the next scheduled dose is:

  • more than 2 days away, administer WEGOVY injection as soon as possible.
  • less than 2 days away, do not administer the WEGOVY injection dose.

Resume dosing on the regularly scheduled day of the week.”

Wegovy’s FDA label, section 2.4

“If a dose is missed, instruct patients to administer MOUNJARO as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day. In each case, patients can then resume their regular once weekly dosing schedule.”

Mounjaro’s FDA label, section 2.1 (Zepbound’s, section 2.3, says the same)

Example: an Ozempic dose 3 days late is inside the label's 5-day window, so the label says to take it as soon as possible and keep your usual day.

How the days are counted

The checker counts calendar days from the day the dose was due to today. On a window’s last day the hours decide it: Mounjaro’s and Zepbound’s labels give theirs in hours too (96), and Wegovy’s patient counseling puts its 2 days at 48 hours. A gap of a week or more means the next dose day has come as well, which the labels’ one-dose rule doesn’t cover; Wegovy’s label adds a rule for 2 or more missed in a row.

Questions

What if I missed two Wegovy doses in a row?

Wegovy’s label covers it: “If 2 or more consecutive doses of WEGOVY injection are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions.” Your prescriber sets the dose you restart at.

Can I move my weekly shot to another day?

Ozempic’s label allows a new day as long as two doses are at least 2 days (48 hours) apart; Mounjaro’s and Zepbound’s, at least 3 days (72 hours). Wegovy’s label asks for the same day each week and gives no rule for moving it.

Is there a rule for compounded semaglutide or tirzepatide, or a research peptide?

No FDA label covers them, so no label rule applies. The instructions of whoever prescribed it are the ones to follow.
